Enrollment Process for MeckCARES

The family/legal guardian calls MeckLINK at (704) 336-6404. MeckINK screens via the telephone; and if the youth meets criteria for medical necessity the family chooses an enhanced benefits provider for diagnosticassessment and care coordination. Or,

The Enhanced Benefits provider submits the MeckCARES Enrollment documents (i.e. request form, consent forms, release of information) to the MeckCARES Enrollment Coordinator as a possible MeckCARES enrollee.

The MeckCARES Enrollment “eligible” procedure begins:

Verification that the Enhanced Benefits provider has completed Child and Family Team trainings 101 and 201 minimally through the MeckCARES Training Institute;

Family/guardian has registered and chosen a date to attend the MeckCARES Family Orientation training.

The family/legal guardian attends the MeckCARES Family Orientation training and receives materials on MeckCARES, to include revisiting the MeckCARES consent forms agreeing to be contacted by and/or to participate in Parent Voice and the UNCC National Evaluation.

Upon completion of the MeckCARES Family Orientation training by the family/guardian, the youth becomes officially “enrolled” into MeckCARES.

The consumer is enrolled into the Participant database and the EDIF (Enrollment Demographic Information site for UNCC) is completed.

The Enhanced Benefits provider works with the family to assemble a Child and Family Team and begin the wraparound process. Additional services and supports are identified at that time and as needed.

A family/guardian who signs the MeckCARES consent to receive information on the UNCC National Evaluation will be contacted directly by the UNCC Evaluation Team. After learning more they may elect whether or not they chose to participate. A family/guardian who has a MeckCARES consumer may also contact the UNCC Evaluation Team directly at 704-687-3513 for further information or to participate.

A MeckCARES representative from TRAC will contact the youth/family within 7 days of enrollment into MeckCARES to obtain baseline data for National Outcomes specific to the SAMSHA System of Care grant initiative.
