The One Child One Plan is a dynamic tool to be used at each monthly Child and Family meeting. It serves as a coordinated service and support document which describes the youth and family, while outlining the work to be undertaken by the entire Child and Family Team to meet the youth and family’s needs in hopes of achieving the family’s vision.
